Help - my mind's gone blank. I'm translating an automated B2B website "selbst-tester" that asks the potential customer a lot of questions, she clicks on the answers and it assesses if she needs the services on offer, and which ones.

What do we call these things in English - online "check-ups" or "questionnaires" - for the life o'me I can't find the right word.

IMO the name depends on the perspective. Sales and marketing
types would call this a lead questionnaire, as its purpose is to help the salesperson identify and contact potential customers (and to have them do the legwork in the guise of 'self-assessment').

'customer profile questionnaire' is another option, and how it might be described to the potential customer.

IMO you should avoid 'self-assessment' and 'self-evaluation' because these terms are commonly used for DIY tests (questionnaires) for personality, aptitude and performance (and your ideal partner...).

Craig: I take your point about their not being customers yet, but IMO this distinction is not often made in common English usage (if only because sales types regard everyone as a customer; it's just a question of when and for what ;-) )

In good sales speak you could call it a prospect profile form, but this is probaly not well established outside sales circles.

  -> Could be (you can see that I was thinking in this direction...). It depends on the content (if the questions ask for everything but the kitchen sink, you could hardly call it a 'needs profile').
