Reference: Captions in US federal courts Reference information: See these "caption" forms published by the US federal courts. (You will see they contain exactly the same type of information as Johanna's helpful reference in German.) However, "captions" in US law would not enumerate the counts of a complaint. Another functional difference may be that it would not ever be considered a separate document. Rather, the information it contains, usually printed in a rigid format, is required to head every document on the docket for that case.
