issue at hand; area needing work; issue to work on
Explanation:
I would suggest you find something along these lines to use or use a mix of all of them. This is the sort of language used by natural or alternative therapists. 'The issue brought to the session' is something I have heard a lot, too. Of course, German finds a way to reduce it to one word, but I've never come across something as simple in English.

Here, for instance, is information about Family Constellations, a therapeutic technique with roots in a variety of modalities. The 'issue' brought to any session is referred to as the 'difficulty' or 'concern'. I think it is extremely important to specifically hunt down the English terminology for the particularl therapeutic modality under discussion. Here is a website describing it:

'A family constellations session typically takes place in a workshop made up of a group of individuals who are not related. Members of the group stand in for family members of the person or couple presenting a difficulty or concern. The person seeking resolution of an issue, who is referred to as the seeker, or the group facilitator chooses these representatives and places them into position as members of the individual's family, also choosing one person to take the place of the seeker in order to complete the family dynamic. The use of other individuals to represent the family is believed to illuminate the disharmony within the family, and the individuals standing in as members of the family are believed to be able to feel and experience the emotions of the person whose role they have taken on. Hellinger, the developer of family constellations, calls this sense of connectedness, which is said to be felt telepathically by members of the group, the morphogenic field.'

This website might be quite useful to you in that it has pages for many types of therapy and you might be able to glean the correct terminology from them.

If this is Family Systems Therapy, you might find the correct terminology here: https://www.betterhelp.com/advice/therapy/what-is-family-sys... They seem, at a very cursory glance, simply to use 'problem' or 'issue'.

agree  Michael Martin, MA: "Issue" will work. The words you added will help to flesh out the concept. But you won't need them for translation purposes.

-> Thanks for your thoughts. I propose these translations because this the language used. Once established, just using ‘issue’ would work, but it depends on the specific modality. Each have their specific terminology.

agree  Vittorio Ferretti: with "issue" only

agree  Ramey Rieger (X): Hi Helen, 'issue' is fine. Still, 'obstacle' is the term we use in family constellation work.//I know. Hellinger followers use 'personal issue.'

-> Hi Ramey, I have a relative who is a trained family constellation therapist and she says there is no set terminology. She herself uses ‘issue for resolution’ but it is quite evident from the links I’ve provided that it differs from therapist to therapist

roadblock

Explanation:
I like 'resolving roadblocks' because the imagery is close to 'Baustelle auflösen' - in both instances, there is unfinished business or obstacles that need to be cleared away for progress to happen.

Compare with this:
"Determine the likelihood that he would be able to talk with someone or go somewhere during a time of crisis. Assess the barriers to using these strategies and identify ways to resolve roadblocks or identify alternatives." https://books.google.com/books?id=c_HEDgAAQBAJ&pg=PA221&lpg=...

obstacle(s)

Explanation:
I have no issue with 'issue', but having participated in many healing processes, especially family constellations, we view this as an 'obstacle' to our true being, which is love. For 'Baustellenbeteiligten' I would use '(effected) family members' since family constellations deal with family members impacted by traumatic events that are passed down or over to other family members.

On the Wiki page for Bert Hellinger, the grandfather of family constellations, the term 'urgent personal issue' is used. So maybe just put a 'personal' in front of Helen's suggestion and you're home free. Still, for Baustellenbeteiligten' you need another term - personally effected family members might work, although very often, the family members do not know they are effected. It's very subtle.
