This is a questionnaire for evaluating the efficacy of a cosmetic product (for pigment spots).

Uniformatisation (0 signifie « uniforme » et 5 « absence d’uniformité »)"

I understand that this has to do with whether the complexion is even or not, I don't like the sound (or look) of the literal English translation here, would evenness do? then, I'd have to be consistent "0 meaning "even" and 5 meaning "uneven/lack of evenness".

I see your problem. Evenness sounds clunky. But can you formulate these open ended questions differently? (how large / what colour / how even in tone). The questionnaire seems to direct at both t overall complexion and patches of pigmentation which makes it harder for you

You're right about "evenness" but In cosmetics, one talks about an *even* complexion (or one that looks even, because the skin has been coated with makeup), not the evenness of the complexion.
