Pijn of dysaesthesie, persisterend langer dan 3 maanden na doorgemaakte herpes zoster. De kans op PHN is groter bij ernstige herpes zoster met veel pijn of prodromale pijn, en bij oudere leeftijd. Circa 5% van oudere patiënten heeft na 1 jaar nog PHN. Zeer moeilijk te behandelen. Antivirale therapie (aciclovir), gegeven in een zo vroeg mogelijk stadium, zou de kans op postherpetische neuralgie kunnen bekorten. Ook adequate pijnstilling (zonodig met opiaten) in het acute stadium zou helpen. Corticosteroïden, vroeger aangeraden, zijn niet effectief. Sommige patiënten reageren goed op anti-epileptica (Tegretol) of tricyclische antidepressiva (amitriptyline). Andere opties zijn gabapentine, fluvoxamine. Zenuwblokkade d.m.v. anaesthetica injecties rond de zenuw of het spinale ganglion (via anaesthesist) zijn soms effectief. Lokale opties: koelen (coldpack), lidocaïne lokaal, capsaïcine crème."
