One more excerpt from the text I'm currently working on, and then we'll (hopefully) call it a day:

Tarkastus suoritetaan näytteenottotaulukon mukaan. Jos tarkastus suoritetaan muualla kuin tavaran vastaanotossa silloin osien mukaan laitetaan seurantalomake AC1.
"

As simple a verb as "laittaa" might be, its meaning is 100% eluding me in this context.

If it means "to put/place", then *where* is the tracking form being placed?
This information is missing from the sentence entirely.

Or, is this a jargon-specialized meaning of "laittaa" that cannot be easily connected to the "put/place" meaning?

Please note it's not just "laitetaan", but "mukaan laitetaan". Laittaa mukaan means "to include; send toghether with" etc.

Of course it may mean fill out, too, (as Spencer said) - at least some fields in the form, but implicitly (not coming from "laatia" or anything). It's not clear from the context where it will be filled out, but I'd guess (as it is a tracking form) that each person would fill in their notes.

Asker: Thanks for responding. Although the text that this comes from has several spelling/punctuation issues, I think the verb here is pretty clearly "laittaa" and not "laatia". There's also at least one other example of "laittaa" being used this way, a bit earlier in the text.
