first-line metastatic colorectal cancer

English term or phrase: first-line metastatic colorectal cancer
I know what "first-line" means when it is used with "therapy", e.g. "first-line treatment for cancer" etc. (i.e. initial treatment).
However, I am somewhat puzzled now as it seems to refer to the disease itself. The phrase is "in patients with first-line metastatic colorectal cancer". There are quite a few clinical study titles & descriptions where this particular phrase is used, e.g. "study evaluating RECENTIN (cediranib) compared with Avastin (bevacizumab) in patients with first-line metastatic colorectal cancer" or "no statistically significant difference in overall survival (OS) in patients with first-line metastatic colorectal cancer". What would be the meaning of "first-line" in these cases?
