podría ser algo así:


evalúe la influencia que ejerce usted, en una escala de 1 a 5 y en la cual el 1 significa que "no ejerzo ninguna influencia en la prescripción del medicamento" "o "qué fármaco se receta"...y así todas las combinaciones que quieras hasta llegar a la que más te guste
