Spanish translation: Monto a cargo del miembro

Hello. I am translating an EOB for a U.S. Spanish audience. The term appears as a header in a table which lists off the patient's purchased drugs at a pharmacy and their respective costs. The other headings, which precede it are "Date of Service," "Reason Code," "Billed by Provider." "Allowed Amount," and "Paid by Plan." The amounts listed under "Member Share Amount" and "Paid by Plan" do not add up to the "Billed by Provider" amount, so that is throwing me off. The best I have come up with for the term in question is "cantidad de la cuota del miembro." Any guidance here would be greatly appreciated.

Explanation:
Although the figures would help, I understand this may be the part of the total that will be charged to/covered by the Member. I am not sure whether it is the part/share of the total amount billed by the provider or of the total amount paid by the Plan, though. Does this make any sense?

If it is a copay, it is a Mutualista, and the Members are the Socios de la mutualista.
