No estoy muy segura del significado de "form" y de "undertaking" y estoy un poco confundida.

Form: me parece que simplemente se refiere al Formulario o aplicación o al contrato/acuerdo que se esté utilizando.
Undertaking: es la empresa/emprendimiento/iniciativa
Pero me parece que debes ver todo el sentido del concepto "accession undertaking".

Personalmente lo dejaría como Contrato de Agenciamiento en el entendido de que el resto del documento y el formulario/contrato mismo debe establecer el objetivo, proceso, condiciones, etc.
