Вот тут объясняется, что эта фраза именно значит:
человек постоянно беспокоится о настоящем (а вдруг мой перевод не понравился, поэтому они молчат), будущем (а вдруг мне никогда не предложат работу), о том, что всё будет плохо (а вдруг мне вообще придётся менять профессию, потому что я настолько переживаю, что партачу всё время, и от меня уйдут все клиенты), он ТАК беспокоится, что "у него голова идёт кругом", "ум за разум заходит"

Any parent will know the well used term “I was worried sick” but is it actually true? Can you worry yourself sick? There is no such thing as being officially diagnosed with a disorder called worry.

Oh, I'm so worried about this story. What if I don't get the information across well enough? What if my boss doesn't like my work? What if that means I lose my job? And more importantly, what if people at home think this sleeping sequence is really lame? According to researchers at the Harvard Medical School, humans are the only species that have the ability to think about the future. And while that has many advantages, it also means they can think about what might go wrong. And that is when people start to worry.
