Explanation:
I know it may look awkward to you compared to one-word source but the thing is your text do not provide information as to what kind of service seniors specifically need or if they also involve medical care.
They may be staying healthy to the end only needing saying hi but also may be already needing to see doctors and take medicine.
I needed to cover both so this look like a chimère. こと makes a noun. 居宅 is originally a word for elders' home care
while I think 在宅 is mainly for visiting medicine services and for patients who are spending lifetime at home.
