Le placebo est une présentation à l’identique du médicament à l’étude mais qui ne contient pas le principe actif. Ainsi lorsqu’un essai randomisé contre placebo est réalisé, un sujet dans l’essai recevra au hasard soit le médicament à l’étude soit une copie de ce médicament, et ni ce sujet, ni le médecin qui le prend en charge dans l’étude, ni la personne (souvent le médecin) qui évaluera le critère de jugement ne sauront ce qui est réellement pris. On parle dans ce cas de mise en insu ou d’aveugle. Les sujets recevant le placebo constituent ce que l’on appelle le groupe témoin (ou groupe contrôle par anglicisme).
