This phrase is from a parish register dating to from 1915 to 1920. The entire sentence reads:

Foraeldrenes fulde Navn efter Forevisning af Dobe- eller i alt Fald Vielseattest eller ved Eftersyn i Kirkebogen; deres Stilling og Bopael; samt for Moderenes Vedkommende Angivelse af hendes Alder med det flydte Aar.

All my attempts at translating this phrase have been clumbsy (as well as the first part of the sentence as you'll see from next posting): "concerning the mother's declaration of age and the age reached"? Any suggestions would be most welcome.

I think that "Angivelse af hendes Alder med det fyldte Aar" is a pleonasm.
And I would use "indicate her age" rather than "indication of her age".
