You could try to work with this, as a UK audience would familiar with the term “pupil referral unit”, which is an approximation of a diagnostický ústav.

Perhaps adapting it to "youth referral unit", e.g. “Working at a Youth Referral Unit with young people aged 11-16yrs with emotional and behavioural difficulties”
