That might be true when they are very small, but both of ours were able to distinguish between German and English by age 2, and by 2.5 even name the language, e.g. describing a German Hallo as German and an English Hello as English. From at least age 2, when they did mix languages it was always because they couldn't think of the word in one language for the moment and thought they might as well try the word from the other language. They'd even use the other language's pronunciation to make it sound like a word from that language. Sometimes this technique even works

This little boy is already age 4 so I'd be surprised if he couldn't tell the languages apart and separate them already.
