@ Fiona I had already posted in the discussion box the answer that I am going to use. If anybody else had already posted an answer, then it would have been appropriate to leave the question open. As nobody responded by the time I found my answer, choosing to close it at that point is not necessarily "bad" practice.

But it's a standard field on a form... ... why do your patient's circumstances matter? It would also be good practice to leave the question open in the event someone comes up with an answer.

Self-declaration I think this may be something to do with travel abroad requirements, as the patient is actually a British person, who is being treated in a hospital in Italy Could this be it I wonder.... in which case I will just leave it as self-declaration, given that that is all the Italian says!

Blood type and screen Now that you've posted the text before and after, the meaning is clearer. It refers to "blood type and screen" - tests performed before surgery. I'm not sure what the "self-declaration" refers to, as a hospital would always want to do their own blood test before surgery to avoid any complications. Maybe another colleague can shed some light.

@ Fiona Yes, this is from a paper checklist entitled "Verifica di completezza della cartella clinica", which seems to be intended for electronic health records in some way I think. It seems to verify that documentation and signatures (by both medics & patient when required) are complete and filled in.
This particular term is a complete phrase on a row, of which the previous and subsequent rows read as follows:
"Valutazione preoperatoria eseguita in prericovero dell'anestesista",
"Autodichiarazione type & screen"
"Consenso al trattamento con emocomponenti/emoderivati"
