I am trying to keep the sentence in one piece, but how to handle the adverbial quality of "de afhankelijk van de ernst van de kwaal" verminderde?

NB: one possibility is of course to add dashes around it, like "the - depending on the severity of the ailment - reduced resistence", but I'm not sure that that 's the way to go.

agree Adam Smith : Probably the best option, but I'd get rid of "extent depending", and simply have "reduced BY the severity of disease", IMHO. :-)

I think this is how to deal with it - otherwise it still remains a very Dutch sentence

These programmes are intended to improve resistance, which has been been compromised / reduced and which is dependent on severity of the disease (or disease severity)....

Bit of a mouthful -but it covers all the facts.
