I think you need to include "sclerotic" in your translation.
Also, an area with increased uptake is also called a "hot spot" on PET. If your target audience are healthcare professionals, you can safely use it. Otherwise, you can say

2. sclerotic lesion with increased uptake ("hot spot"), or
2. sclerotic lesion with increased uptake/metabolism

: Does not solve the contradiction mentioned: 'Mehrspeicherung' vs 'ohne FDG-Speicherung'. One would not necessarily call this a 'hot spot'.
