Medical vs. Treatment If البطاقة العلاجية = Medical card, then what is the equivalent of البطاقة الطبية?

I think a medical card contains the medical history and a brief description of the illness of the bearer, usually carried by people with chronic or dangerous medical conditions. While, on the other hand, a treatment card authorizes a medical facility to provide medical treatment to an employee. As usual, of course, I stand to be corrected learned colleagues.
