I think we neglected to point to Ethan (since we just focused on the "non-study" term) the fact that the Arabic text construction is absurd and الإستبيان is misspelled.
I believe the case is that the testing body had an English source text that he handed to an incompetent Arabic translator, and he wanted the applicants to translate it back to English.

I like the final translation you decided on. But I maintain that by "passive" I mean you have received a visit, and any other understanding would shake my confidence in my native-speaking abilities.

We agree that a visit has happened because this is the passive voice. All I'm saying is that it is unknown by this phrasing if the patient was visited by someone else or performed the visit himself (visited a hospital), hence, "Have you had a non-study visit" is the correct option here.

I second Nesrin's opinion, "Have you been visited ..." it can't mean anything else in any Arabic dialect.

Hi Ethan. I would be HIGHLY surprised if هل تمت زيارتك زيارة... was to mean "Did you undertake a visit...". The only way that this sentence can be understood to mean that you actively visited is if in English it meant "has your visit been concluded/ has your visit come to an end?" But even then you'd say هل تمت زيارتك غير الدراسية not هل تمت زيارتك زيارة غير دراسية. But anyway, as long as the final translation can be understood correctly, that's fine.

Nesrin. Just wanted to let you know that now, having received the corrected assignment back, it looks like my initial thought - "Have you had a non-study visit..." was the correct one, since it is indeed unclear whether the patient went to the healthcare provider or the healthcare provider went to the patient. تمت زيارتك just means the a visit happened, but no more than that.

I understand how it can be confusing because another form of the verb - أتم - means to conclude, to complete. However if you wanted to use it to say "have you completed a visit" you would have to say هل أتممت زيارة i.e. conjugating أتم for the subject. But it would sound a bit unusual.

Just simply هل زرت or هل قمت بزيارة... But تم + مصدر is always a passive construction, e.g. تم بناء الأهرامات = بنيت الأهرامات

No Ethan, تمت زيارتك here is clearly a passive construction, i.e. "you have been visited" or "you have received a non-study visit".

Nesrin - I think you are right in the non-study visit definition, I was able to see that on two of the links (the others were inactive for some reason). However, I'm unsure about the fact the patient is being visited, in which case Sktrans may also be right. Of course I know what تم means, but what exactly would it mean here تم زيارتك  etc. - Maybe "Have you been to/Have you had a non-study visit since your last study visit?" or - "Have you been to see a non-study doctor since last seeing a study doctor?"

I'm not entirely sure about the context, but you do mention "Health Resource Utilization Questionnaire" - so I think it means that the respondent is visited by people conducting the study.
